The ball can disappear from play this way:
1) The ball carrier is in a precarious position
2) A bomb is thrown at the ball carrier
3) He tries to catch the bomb
4) He fails precarious position and falls into a pit
5) The ball disappears with him, it does not scatter and appear from a teleport pad as it should
This happened to me today, the replay is attached.
