It mentions that if a player falls into the lava on your turn it is a turnover. This does not happen

I've seen this too.

It seems that failure of a "precarious position" roll does not cause a turnover at all - which it should do.

Another problem with Lava is the injury that it causes.

I'm pretty sure that it should be an automatic casulty (being a 'deadly trap')

but I have had quite a few KO's and Stuns.
