I guess a good design can be this :
Lethal Trap => Injury Roll (from badly hurt to death) if no apo, can't return.
Non Lethal Trap => Like Pushing in the crowd on normal Blood Bowl
(so if stun => reserve box, if Ko => need a sponge, if Injured => need apo, if not, can't return)
Apo works for injury like in Blood Bowl, if the result is badly hurt => reserve box.
Someone who falls into water can get back, see the design of my dungeon "Gog and Magog's Tomb" and you will see that in therm of roleplay/map design, it's a good compromise.
